We built a Functional Neurological Symptoms questionnaire for the Devon Partnership Trust as an intranet app. DPT provide mental health and learning disability services across the whole of Devon.

MyFNSDiary asks ICD9 questions and records responses.

Patient names come from the Care Notes API.

There’s a summary screen of who’s seen what questionnaires and how far they are completed.

All responses over time are stored: so we can track the patient’s progress.
